MedChartScan Inc. builds clinical-intelligence software for risk adjustment and documentation quality. Healthcare organizations use it to find the diagnoses and care gaps their records already support, to validate notes while they are being written, and to defend what they submit when an audit arrives.

What we build

One platform carries a patient's documents from ingestion to a defensible claim. Faxes, scanned charts, prior records, labs and PDFs come in automatically; ranked, evidence-backed suggestions go to the people who decide; accepted findings are written back to the EHR with the trail that proves them.

  • SideCar. An on-the-glass extension that works inside the EHR a clinician already uses. Coding and care-gap intelligence appears on the chart itself, with no portal, no tab switching and no second login.
  • SideCar Enterprise. A workspace for coders, auditors and administrators: evidence-first chart review, admin and analytics, and ingestion of payer open-gap files.
  • Audit Module. A RADV-first audit workflow. Every accepted suggestion is linked to the submitted claim and the exact passage of the medical record that supports it, so the documentation package is ready when an audit letter arrives.
  • E/M Coding. A deterministic audit of office and outpatient E/M levels against the CMS rules, with anti-overcoding guardrails. You can run it on your own note in the live demo.

The evidence is the product

Every suggestion points at the document passage that supports it and explains the reasoning in plain English. That is what makes a submitted code defensible, and it is the reason to trust the platform at all.

A person makes the decision

The platform surfaces and ranks; a clinician or coder accepts or rejects. Each decision is recorded with the evidence it rested on, which is where the audit trail comes from.

We show mechanisms, not promises

The site carries an interactive SideCar demo built from the extension itself and a live E/M audit you can run on your own note. Where a number is not proven, we show how the product works instead.

Built for the clinic as it is

The work is covered by Business Associate Agreements under HIPAA, supports the CMS-HCC V28 model alongside legacy models, and is designed to run on the hospital workstations and virtual desktops clinicians actually have.
